Job summary

Rushford Center, part of Hartford HealthCare, is seeking a Recovery Support Specialist to build trusting relationships with patients in recovery and mentor them toward ongoing wellness. You will link clients with community resources and collaborate with housing and transportation supports to aid successful transitions back into the community.

Ideal candidates have peer support or mental health experience and personal recovery insights, with willingness to pursue Peer Support Specialist

Qualifications

• High School diploma or equivalent; related experience welcomed.
• Requires current CT driver's license; must qualify for Public Service License.
• Demonstrated peer support or mental health experience.
• Personal recovery experience and willingness to pursue Peer Support Specialist certification.

Responsibilities

• Assess needs with clients and develop an Individualized Recovery Plan (IRP).
• Use recovery-based interventions to progress toward IRP goals.
• Educate and consult the Clinical Support Team on client-centered approaches.
• Provide illness and recovery management education to clients and families.
• Engage natural support networks and activities for community integration.
• Coordinate with Housing Supports to help clients secure safe, affordable housing.
• Arrange transportation to clinical, medical, and other appointments as needed.
• Maintain program data, progress notes, and releases as required.
• Document to meet DMHAS PR program and funding requirements.
