Job summary

Rushford Center, part of Hartford HealthCare, seeks a Recovery Support Specialist to build trusting relationships with patients in recovery and mentor them on community reintegration.

You will create IRPs, provide recovery coaching, and connect clients with housing, transportation, and support networks while maintaining documentation and coordinating with clinical staff. A valid CT driver's license is required.

Qualifications

• High School diploma or equivalent with relevant population experience.
• Valid CT driver's license and eligibility for Public Service License.
• Demonstrated peer support or mental health experience.
• Personal recovery experience relevant to services provided.
• Willingness to obtain Peer Support Specialist certification.
• Able to travel daily among Rushford sites and transport clients when needed.

Responsibilities

• Assess needs with client and develops an Individualized Recovery Plan (IRP).
• Use recovery oriented interventions to progress toward IRP goals.
• Provide education to the Clinical Support Team on client-centered approaches.
• Instruct and coach clients and families in illness and recovery management.
• Engage natural support networks and community activities for integration.
• Collaborate with Housing Supports to help clients secure housing.
• Arrange transportation for clinical and other appointments.
• Maintain program data, reports and client records per funding requirements.
• Document progress and releases; work under supervision of a licensed clinician.
• Perform other duties as required.
