Southern California Health and Rehabilitation Program (SCHARP) seeks a peer mentor/case manager to support individuals in recovery. Under supervision, you will apply peer-counseling methods, assist with linkage to services, and escort clients to resources in the community.

You will share your lived experience to inspire hope while working as part of a team to coordinate care, attend trainings, and ensure confidentiality in line with HIPAA policies.

Competenze

• Must have experience with and managing a psychiatric disorder and knowledge of 12-step recovery.
• Lived experience with mental health, justice involvement or homelessness is required.
• Must pass California DOJ background check and Live Scan.
• Valid California driver’s license and car with adequate insurance preferred.

Mansioni

• Takes direction from experienced staff for peer counselor interventions per recovery plans.
• Provides case management and escorts consumers to indicated resources.
• Escort consumers to detox, recovery homes and sober living environments.
• Share personal recovery experience to offer hope and attract participants.
• Participate in trainings and meetings; ensure HIPAA compliance.
• Coordinate with supervisors and partners, maintaining confidentiality.
