Job summary

Harbor Community Health Centers seeks a Peer Recovery Support Specialist to provide individualized recovery support, advocacy, and care coordination for individuals in substance use recovery services.

Preference will be given to candidates with lived recovery experience. You will work with interdisciplinary teams to develop recovery plans, support engagement, and navigate insurance and scheduling needs while maintaining confidentiality and professional boundaries.

Qualifications

• Minimum 1 year of experience in peer recovery support or related recovery settings.
• Lived recovery experience preferred.
• Current CPS credential or eligibility to obtain certification.
• Current First Aid/CPR certification or ability to obtain certification.

Responsibilities

• Provide individualized recovery support and case management in collaboration with clinical teams.
• Assist with insurance navigation, referrals, and appointment scheduling.
• Administer SUPRT surveys and track follow-up as required by program and grant guidelines.
• Document interactions and maintain confidential patient records per organizational standards.
• Uphold professional boundaries, HIPAA compliance, and HarborCHC policies.
