Responsibilities

The Licensed Practical Nurse in Home and Community-Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies. The RN coordinates care plans with other healthcare professionals and ensures continuity of care through communication with physicians, agency staff, patients, families, and community resources.

Education and Experience

Deliver high-quality clinical services within your scope of practice, infection control standards, and the care plan. Conduct clinical assessments as required by federal/state programs and payers. Verify patient eligibility and medical necessity for services. Develop and update individualized care plans in coordination with community providers, ensuring they meet patient needs and are approved by physicians.

Experience Desired

Minimum of one year RN experience; PEDS experience preferred.

License Requirements

• Current RN licensure in the state of practice
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation
• Current CPR certification
