Responsibilities

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care through effective communication with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

Deliver high-quality clinical services within your scope of practice and infection control standards, following the care plan and coordinating with the care team from admission to discharge.

Complete clinical nursing assessments as required by federal and state programs and payers.

Verify patient eligibility and medical necessity for services according to payer policies.

Develop and update individualized care and service plans in collaboration with community providers, ensuring they meet patient needs and are approved by physicians.

Adjust care plans as needed, based on patient needs and with physician approval.

Experience and License Requirements

A minimum of one year of RN experience; PEDS experience preferred.

Current RN license in the state of practice.

Valid driver’s license, vehicle insurance, access to a reliable vehicle or public transportation, and current CPR certification.
