Responsibilities

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes as prescribed by the physician or authorized by the state/program, complying with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and helps maintain continuity of care by planning and sharing information with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the care plan and in coordination with the care team from admit to discharge.

Completes clinical nursing assessments as required by federal and state programs and payers.

Ensures patient eligibility and medical necessity for services as defined by payer and agency policy.

Develops and revises individualized care and service plans according to federal and state requirements with community providers.

Ensures the plan of care's frequency and duration meet patient needs, initiating revisions with physician approval as needed.

Experience and License Requirements

Experience Desired: Minimum of one year RN experience; PEDS experience preferred.

License Requirements: Current RN licensure in the state of practice, valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation, and current CPR certification.
