Responsibilities

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care by planning and exchanging information with physicians, agency personnel, patients, families, case managers, and community resources.

Education and Experience

Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care/service plan, and in coordination with the care team from admission to discharge.

Completes clinical nursing assessments as required by federal and/or state program requirements and payer guidelines.

Ensures patient eligibility and medical necessity for services based on payer source and agency policy.

Develops and revises individualized care and service plans according to federal and/or state requirements with community providers.

Ensures care plan frequency and duration meet patient needs and revises plans as needed with physician approval.

Experience Desired

A minimum of one year RN experience; PEDS experience preferred.

License Requirements

Current RN licensure in the state of practice.

Valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation.

Current CPR certification.
