Job summary

The University of Florida on the Jacksonville Campus is seeking a Clinical Case Manager I- Peds Early Steps to conduct screenings, family assessments, and coordinated case management as part of a multidisciplinary team.

The role supports eligibility determinations, updates IFSPs, coordinates services, and connects families with community resources under IDEA Part C; a Master’s or Bachelor’s degree with related experience is required.

Qualifications

• Master's degree in an appropriate area; or a Bachelor's degree in an appropriate area of specialization and two years of relevant experience
• Valid Driver's License
• Be eligible for a Medicaid Provider Number, and enrollment with Children's Medical Services.
• Requires Level II Criminal Background Screening.

Responsibilities

• Conduct screenings, complete family assessments, provide case management activities, and serve on a multidisciplinary team to determine eligibility during evaluations.
• Update IFSPs, coordinate services, make recommendations, and access community resources for children referred to Northeastern Early Steps through IDEA Part C.
• Participate in/conduct developmental evaluations and family assessments in collaboration with the multidisciplinary team for intake, eligibility, progress, IFSP updates, and intervention planning.
• Maintain current and accurate client records including plans, notes, and billing information timely and accurately.
