Qualifications

• Licensed PT in Florida per 468. Florida Statutes, and Rule Chapter 64B20, F.A.C.
• Must have a valid Driver’s License and be eligible for a Medicaid Provider Number.
• Must enroll in Early Steps online orientation modules within 90 days of application date.
• Enrollment with Children’s Medical Services required.
• Completion of Early Steps orientation modules (1-3) within 90 days of application date; CEU/In-service hours requirements.

Responsibilities

• Completes developmental evaluations, screenings, and assessments with multidisciplinary team to determine eligibility and plan intervention.
• Provides intervention/consultations as authorized on each child’s IFSP using the Primary Service Provider approach.
• Develops Plan of Care and obtains authorization for services from third party payers and program policies.
• Maintains clinical materials, supplies, and work area; requests replacements as needed.
• Ensures thirty-six (36) hours of CEU/In-service training every three years; aligns with early intervention competencies.
