Classification Minimum Requirements: Master's degree in an appropriate area; or a Bachelor's degree in an appropriate area of specialization and two years of relevant experience
Under the direction of the Service Coordinator Supervisor, the Clinical Case Manager is responsible for conducting screenings, completing family assessments, providing case management activities, and serving as a member of a multidisciplinary team to determine eligibility during evaluations. Responsibilities also include updating IFSPs, coordinating services, making recommendations, and accessing community resources for children referred to the Northeastern Early Steps Program through Part C of the Individuals with Disabilities Education Act (IDEA). Services are provided in natural environment settings, with responsibility for maintaining compliance with Local, State, and Federal guidelines, including requirements for best practice and billing established by AHCA and the Early Steps State Office.
Participates in/conducts developmental evaluations/screenings, consultations, direct services, and family assessments in collaboration with the multidisciplinary team when appropriate for the purpose of completing intake, determining eligibility, monitoring progress, updating IFSPs, and planning/providing intervention.
Works with IFSP team to ensure State and Federal timelines are met with regard to timely service delivery, transition, IFSP development/updates, and LEA/SEA notification.
Recommends, coordinates, and makes referrals to community services to meet identified outcomes on Individualized Family Support Plans.
Maintains current and accurate client records including program plans, intervention notes, case notes, and billing information timely and accurately for payment.
Participates in local and statewide meetings and trainings to ensure best practice and compliance; Completes trainings/modules as required by the program and organization.
Expected Salary:
$46,000 - $55,000
Required Qualifications:
Master's degree in an appropriate area; or a Bachelor's degree in an appropriate area of specialization and two years of relevant experience
Valid Driver's License
Be eligible for a Medicaid Provider Number, and be eligible for enrollment with Children's Medical Services.
Requires Level II Criminal Background Screening.
Preferred: Bachelor's degree in Early Childhood or Early Childhood Special Education, Social Work, Psychology, or Sociology. Two years of appropriate experience with children birth to five with special needs.
