Qualifications

• NYS RN License required.
• Strong clinical documentation skills.
• Familiarity with CMS billing regulations.
• Home health or community care experience preferred.
• Excellent verbal and written communication skills.

Responsibilities

• Ensure the accuracy, completeness and integrity of documentation completed by the COC.
• Ensure OASIS assessments are completed per CMS guidelines (OASIS-C Chapter 3).
• Maintain documentation standards within the care team.
• Collaborate with clinical staff to support quality patient care.
