Job summary

Angels of Care Pediatric Home Health in Orlando, FL is hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children during critical transitions to home care.

This role requires high-acuity pediatric care experience, including trach, vent, G-tube, and emergency management. You will ensure safe discharges, prevent readmissions, and provide interim coverage until permanent staffing is established.

Qualifications

• Active, unrestricted RN or LVN/LPN license in Florida.
• Current CPR certification.
• Pediatric home health experience.
• Experience with Trach care, Ventilator management and TPN.
• Ability to care for high-acuity pediatric patients.

Responsibilities

• Provide skilled pediatric nursing 1:1 in patients' homes.
• Assess patients and administer medications, treatments, and therapies.
• Support hospital-to-home transitions and readmission prevention.
• Educate families and caregivers; coordinate discharge planning.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond promptly to emergencies and changes in patient condition.
