Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) or Licensed Practical Nurses (LPN) to join our Pediatric Home Team (Transitional Care Team), supporting medically fragile children during hospital-to-home transitions. You will provide 1:1 pediatric nursing in patients' homes, perform assessments, administer medications, and help with discharge planning and care coordination.

This is a full-time, field-based role with local travel, day and night shifts, on-call coverage, and a

Competenze

• Active, unrestricted RN or LPN license in NC.
• Current CPR certification.
• Experience with high-acuity pediatric care (trach, vent, G-tube) preferred.
• Ability to lift and assist pediatric patients.

Mansioni

• Provide skilled pediatric nursing 1:1 care in patients’ homes.
• Perform patient assessments and ongoing evaluations.
• Administer medications, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond appropriately in emergency situations and changes in patient condition.
