Job summary

Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children and families during critical transitions to home care.

This role emphasizes high-acuity pediatric care, trach/vent care, and emergency management, with a focus on safe discharges and continuity of care until permanent staffing is established. Biweekly pay, day/night shifts in Harrisburg, PA.

Qualifications

• Active, unrestricted RN or LVN/LPN license (PA).
• Current CPR certification.
• Experience in pediatric home health and high-acuity patients.
• Experience with Trach care and Ventilator management.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients' homes.
• Perform patient assessments and ongoing evaluations; administer meds, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond appropriately to emergencies and changes in patient condition.
