Job summary
Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) to support medically fragile children in their transition to home care. This field-based role focuses on 1:1 pediatric care in patients’ homes, including trach and vent management.

Qualifications
- Active, unrestricted RN or LVN/LPN license (PA)
- Current CPR certification
- Experience with Trach care and Ventilator management
- Pediatric home health experience
- Ability to lift and assist pediatric patients
- Experience with high-acuity pediatric patients
Responsibilities
- Provide skilled pediatric nursing 1:1 care in patients’ homes
- Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies
- Support hospital-to-home transitions for high-acuity patients
- Educate and support parents, families, and caregivers
- Assist with discharge support and care coordination
- Train and support permanent field nurses during transitions
- Provide first-shift and interim coverage as needed
- Collaborate with Clinical Care Managers and interdisciplinary teams
- Respond appropriately in emergency situations and changes in patient condition
