Job summary
Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children and their families during critical transitions to home care.
This role requires high-acuity pediatric care experience, including trach, vent, G-tube, and emergency management, with responsibility for safe discharges, readmission prevention, interim coverage, and continuity of care until permanent staffing is in
Qualifications
- Active, unrestricted RN or LVN/LPN license (PA)
- Current CPR certification
- Experience with Trach care and Ventilator management in pediatric settings
- Ability to lift and assist pediatric patients
- Experience with high-acuity pediatric patients
Responsibilities
- Provide skilled pediatric 1:1 in-home nursing care
- Perform patient assessments and ongoing evaluations
- Administer medications, treatments, and therapies
- Support hospital-to-home transitions for high-acuity patients
- Educate and support parents, families, and caregivers
- Assist with discharge support and care coordination
- Train and support permanent field nurses during transitions
- Provide first-shift and interim coverage as needed
- Collaborate with Clinical Care Managers and interdisciplinary teams
- Respond appropriately to emergencies and changes in patient condition
