Job summary

Therapy 2000 in Texas is seeking a qualified Pediatric Speech-Language Pathologist for home-based therapy. Qualifications

• A current/active license to practice Speech Language Pathology in Texas.
• 3+ years as a pediatric Speech Language Pathologist.
• Experience in a home-health setting.
• Experience or certifications in feeding therapy or other clinical specialties (VitalStim, SOFFI, SOS, etc.)

Responsibilities

• Perform comprehensive Speech Therapy evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with caregivers.
• Collaborate with other providers to coordinate care for patients.
• Complete minimum weekly patient visits as required by the agency.
• Maintain timely chart updates, progress summaries, and logs.
• Attend meetings to align expectations and standards for patient care.
• Adhere to agency policies and state/federal regulations in home-health pediatric SLP.
