Therapy 2000 is seeking a licensed Pediatric Speech Language Pathologist for home-based services in Texas. You will evaluate patients, design individualized treatment plans, and partner with families to implement home programs.

You will coordinate care with a multidisciplinary network, see a defined patient caseload weekly, and ensure timely documentation while delivering high-quality pediatric speech-language pathology services at patients’ homes.

Formación

• Current Texas SLP license.
• 3+ years of pediatric Speech Language Pathologist experience.
• Experience in home-health setting.
• Certifications in feeding therapy (VitalStim, SOFFI, SOS) preferred.

Responsabilidades

• Perform comprehensive Speech Therapy evaluations using standardized tests to determine therapy needs.
• Develop and implement treatment plans aligned with physicians’ orders.
• Create home exercise programs with caregivers.
• Collaborate with other providers to coordinate care for patients.
• Meet a minimum number of patient visits weekly as required by the agency.
• Maintain timely chart updates, progress notes, and call logs.
