Job summary

Abrazo West Campus Hospital seeks a Social Worker to coordinate care, assess transition needs, and promote safe discharge while preventing readmissions. The role integrates case management standards and collaborative care, overseeing transitions and documentation with physicians, patients, and families.

The position requires a Master’s of Social Work and an Arizona LMSW license, with MIDAS and Curaspan training due within 90 days. Preferred: 2 years in an acute hospital and ACM certification.

Qualifications

• Master's of Social Work required.
• Arizona LMSW license required; MIDAS and Curaspan training to be completed within 90 days of hire.

Responsibilities

• Complex psycho-social transition planning, assessment, reassessment and intervention.
• Assistance with adoptions, abuse and neglect cases, including assessment, intervention and referral to appropriate agencies.
• Care coordination and overseeing transition plans.
• Lead multi-disciplinary patient care conferences including Complex Case Review.
• Other related tasks as assigned.
