Phoenix Children’s is seeking a licensed Master Social Worker for a PRN Social Work role within Care Management. The position supports patients and families through bio-psychosocial assessments, treatment planning, and transition planning, working as part of an interdisciplinary team.

Responsibilities include conducting psychosocial assessments, coordinating care, and providing therapy as needed. Requires AZ LCSW/LSW licensure, CPR/First Aid, and strong communication skills to support discharge

Qualifications

Position Summary

This position provides clinical assessment, therapeutic interventions, care coordination, transition planning and emotional support to patients and families at Phoenix Children’s within their scope of practice. Participates as an interdisciplinary team member providing interventions with the patient/family that include a range of care from bio-psychosocial assessment, diagnosis, treatment planning, therapeutic interventions, crisis interventions, coordination of care, bereavement support, and transition/discharge planning. The position requires knowledge of family systems and interpersonal dynamics/relationships and is able to use that knowledge to assist patients and families in achieving therapeutic goals. Knowledge of mandated reporter responsibilities; use of financial, community and other resource networks; coordination with DCS, Police jurisdictions, and other governmental authorities; ability to advocate with outside systems; and, collaborate with medical staff. The position may provide individual, group, family therapeutic interventions for an assigned caseload. The position is responsible for implementation of patient treatment assignments, management of open patient records, and for collaborating with outside referral sources as applicable.

Position Duties
  • Psychosocial assessments and interventions – assess bio-psychosocial status including mental health status, diagnostic impressions, support systems, safety issues, risk for abuse/neglect, suicide risk, evaluate clients for placement appropriateness, coping skills, family systems, and cognitive abilities.
  • Identify therapeutic goals to assist patient and family in improving family functioning, mental health, goal attainment, self-sufficiency, and medical adherence.
  • Identify barriers to treatment, build strengths and protective factors to assist with discharge success; collaborate with interdisciplinary team to set treatment plan goals and objective, facilitate timely and efficient discharges, and participate in proactive transition and discharge planning.
  • Document clinical records promptly to coordinate care, confirm demographics, and provide recommendations for disposition planning as required.
  • Complete handoff documentation to Social Work Department per procedures.
  • Develop and coordinate comprehensive treatment plans and therapeutic interventions for patients.
  • Maintain effective communication with medical staff, nursing, case managers, peers, and families to facilitate coordination of care and continuity of care.
  • Initiate and/or attend care conferences, interdisciplinary rounds, and critical incidents/stress debriefings as necessary.
  • Perform care coordination and monitor patient transitions between outpatient and inpatient services.
  • Refer cases identified as risk management, peer review, or quality issues to appropriate units.
  • Participate in hospital and department initiatives to improve care delivery methods.
  • Conduct or facilitate individual, group, and family therapy meetings, including process and didactic groups; hold regular updates with families and provide education on chemical dependency, behavioral health, and relationship issues.
  • Review open patient records to ensure policy compliance and service delivery per treatment plans.
  • Develop home contracts, level contracts, goals, and objectives as needed.
Education
  • Master’s in Social Work from an institution accredited by the Council of Social Work Education or an equivalent foreign degree.
Experience
  • Child protection experience.
  • Preferred: Three years post-Master’s social work experience in a health or behavioral health setting.
  • Preferred: Pediatric social work experience.

Additional Requirements
  • Current State of Arizona Licensed Master in Social Work by the Arizona Board of Behavioral Health Examiners.
  • CPR/First Aid and CPI training or ability to obtain at hire.
  • Preferred: Valid AZ Department of Public Safety Fingerprint Clearance Card.
