Job summary

Tucson Medical Center is seeking a Clinical Social Worker to interview patients and families, assess home environment and bereavement needs, and coordinate post-discharge planning. You will serve as liaison among medical teams, patients, and community resources to resolve social barriers to care.

The role requires a Master's in Social Work and relevant clinical experience; licensure preferred. You will organize spiritual services, support crisis situations, and help patients navigate

Qualifications

• Master's degree in Social Work or related field required.
• For Hospice, MSW required; licensure as LCSW or LMSW preferred.
• Two years of social work experience in an acute care setting preferred.
• HeartSaver First Aid/CPR or Basic Life Support (BLS) certification required per department; AZ fingerprint clearance may be required per department.

Responsibilities

• Interviews patients and family members to obtain information about home environment, health history, and patient’s personality traits.
• Plans, organizes, directs and schedules spiritual programs and services that meet the needs of patients/families, hospital employees, and medical staff.
• Consults with hospital staff regarding hospital ministry and collaborates with care team to identify and resolve problems.
• Acts as a liaison between patient, family, and hospital staff during crises.
• Assists in developing and implementing the patient’s care plan.
• Interprets illness and treatment information to patients and families; participates in planning conferences.
• Plans for needed care with nurse practitioner, physician, patient, and family.
• Introduces programs for the elderly after discharge and arranges post-discharge services.
• Facilitates liaison between medical staff, patients, families, and community for post-discharge care.
