Job summary

Tenet Healthcare in Goodyear, AZ seeks an experienced Social Worker to coordinate transition plans and assess psycho-social needs for safe discharge. You will lead a multidisciplinary team, educate patients and families, and ensure regulatory compliance.

Requirements include an MSW and AZ LMSW licensure within 90 days, plus MIDAS and Curaspan training. Acute hospital experience is preferred and engagement with complex cases is expected.

Qualifications

• Master's degree in Social Work required.
• AZ LMSW license required within 90 days of hire.
• MIDAS and Curaspan training within 90 days of hire.

Responsibilities

• Assess patients for transition needs and identify those at risk for readmission.
• Conduct complex psycho-social assessments and interventions to promote timely throughput and safe discharge.
• Lead care coordination and ensure appropriate level of care and sequence.
• Educate physicians, patients, families and caregivers.
• Lead multi-disciplinary patient care conferences including Complex Case Review.
• Oversee implementation of transition plans.
• Assist with adoptions, abuse and neglect cases with agencies as needed.
• Other related tasks as assigned.
