Job summary

Qualifications

• Bachelor degree in nursing from an accredited university.
• Preferred: Master’s in Nursing; Case management certification is preferred.
• Current BLS; CA RN license required; willingness to obtain licenses as needed.

Responsibilities

• Deliver care coordination in the clinic setting.
• Plan and deliver family-centered patient care with a multidisciplinary team.
• Advocate for optimal health, access to care and resources.
• Coordinate with physicians, nurses, social workers, therapists, vendors and community partners.
