Job summary

Advocate Health in Charlotte, NC seeks a Pediatric Outpatient Rehabilitation Physical Therapist to evaluate patients and develop responsive treatment plans across outpatient clinics. You will work with a multidisciplinary team to support pediatric functional goals and provide patient education and discharge planning.

The role requires a Bachelor’s degree in Physical Therapy, a state license, and BLS. Weekend/holiday coverage is required and full benefits accompany the position.

Qualifications

• Bachelor’s degree in Physical Therapy program required.
• Applicable state license for Physical Therapy required.
• BLS required per policy guidelines.

Responsibilities

• Evaluates patients, selects and administers assessment techniques, synthesizes information to establish appropriate patient care plan to include necessary modalities, procedures, exercise programs and education as warranted by diagnosis, age and discharge needs.
• Gathers and interprets data from medical records, physician reports, patient, and family as evidenced by documentation.
• Establishes functional goals with patients and provides evidence-based intervention to work toward meeting these goals with ongoing plan adjustments.
• Documents evaluations, tests, progress notes, treatment notes, attendance, education, outcomes, discharge notes and charges as needed.
• Communicates evaluations and progress to physicians, referral sources and other clinical staff; coordinates schedule and treatment programs and discharge planning.
