Job summary

KidsCare Home Health in Travelers Rest, SC is seeking a Physical Therapist to deliver pediatric home-based physical therapy. You will evaluate patients, organize plans, and administer medically prescribed treatments in the home setting.

You will train families and caregivers, coordinate with office staff and other disciplines, and help patients progress toward discharge while maintaining strong documentation.

Qualifications

• Master’s Degree in Physical Therapy required.
• Current state-issued professional license.
• Valid state driver’s license.
• CPR.
• Pediatric clinical experience.
• Home health experience.

Responsibilities

• Evaluate and conduct pediatric home physical therapy as prescribed.
• Provide training to patients and caregivers and assist with discharge planning.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with patient and family regarding progress and home programs.
• Maintain appropriate records and documentation; participate in QA activities.
