Job summary

KidsCare Home Health is seeking an enthusiastic Physical Therapist to provide pediatric home health services. You will evaluate and treat children in their homes, coach families, and collaborate with a multidisciplinary team to optimize outcomes.

The role emphasizes mentoring, leadership pathways, and a balanced workload while delivering compassionate care to children with special needs.

Qualifications

• Master’s Degree in Physical Therapy required.
• Current state PT license required.
• Valid state driver’s license.
• CPR certification required.
• Pediatric clinical experience required.
• Home health experience preferred.

Responsibilities

• Examine, evaluate, and administer remedial treatments for pediatric patients.
• Provide clinical treatment including screenings, evaluations, direct treatment, and family/caregiver training and counseling regarding status, progress, goals, home programming and discharge planning.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with the pediatric patient and family regarding progress, problems, home programs, and other issues related to therapeutic process.
• Refer patients and family members to appropriate services including other professionals, agencies, and resources as appropriate.
• Consult with physician on patient progress and maintain appropriate records.
• Responsible for maintenance of supplies and equipment.
• Participate in Quality Assurance and Performance Improvement (QAPI) program.
• Attend Continuing Education activities and expand scope of service.
• Maintain adequate caseload with appropriate productivity.
• Collaborate with others through knowledge and skills sharing.
• Mentor students and new hires.
• Demonstrate initiative and independent judgment in performing all job responsibilities.
• Serve as backup service provider for other caseloads as needed.
