KidsCare Home Health in Greenville, SC is seeking a licensed Physical Therapist to provide pediatric home health services. You will evaluate, treat, and educate children and their families in the home setting, developing individualized plans of care.

Qualifications

• Must have a Master's in Physical Therapy and active state license.
• Current CPR certification.
• Pediatric clinical experience preferred.
• Home health experience a plus.

Responsibilities

• Examine, evaluate, and administer pediatric physical therapy in the home health setting.
• Provide caregiver training and family education on status, goals, home programs, and discharge planning.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with patient and family regarding progress and problems.
• Refer patients and families to appropriate services and document progress.
• Consult with physician on patient progress and maintain records.
• Maintain supplies and equipment and participate in QA/QI activities.
• Attend continuing education to expand service scope and capabilities.
