KidsCare Home Health is seeking a Pediatric Physical Therapist to provide in-home therapy services. You will evaluate, plan, and direct pediatric patient care, educating families to support continued progress in the home setting.

Qualifications

• Master's Degree in Physical Therapy required.
• Current state-issued professional license.
• Valid state driver license.
• CPR certified.
• Pediatric clinical experience preferred.
• Home health experience preferred.

Responsibilities

• Evaluate and treat pediatric patients in home health.
• Direct patient and caregiver in tasks to restore and improve function.
• Coordinate care with office staff and other disciplines.
• Maintain communication with patient and family regarding progress and home programs.
• Refer patients to appropriate services and resources.
• Consult with physician on patient progress and maintain records.
• Maintain supplies and equipment.
• Participate in QA and continuing education activities.
• Maintain caseload with productive scheduling.
• Collaborate with team and mentor students/new hires.
