Job summary

Therapy 2000 in Texas is seeking a Pediatric Physical Therapy Assistant to deliver home-based PT services under the supervision of a licensed Physical Therapist. You will implement treatment plans, educate families, and coordinate care across the patient’s medical team.

Responsibilities

• Perform comprehensive Physical Therapy treatments under Supervising Physical Therapist goals and treatment plans.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of patients’ medical neighborhoods, coordinating care between providers and disciplines.
• Complete a minimum number of patient visits weekly as required by the agency.
• Meet deadlines for chart updates, progress summaries, call logs, and related paperwork.
• Collaborate with agency personnel to align with expectations and standards for high-quality care.
• Adhere to all policies, regulations, and laws applicable to home-health pediatric PT.
