Qualifications

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine patients’ needs for therapy.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of patients’ medical neighborhoods, coordinating care with providers and disciplines.
• Meet a minimum number of patient visits weekly as required by the agency.
• Complete chart updates, progress summaries, call logs and other paperwork on time.
• Collaborate with agency personnel to uphold high-quality patient care standards.
• Follow agency policies, procedures and applicable laws for home-health pediatric therapy.
