Job summary

THERAPY 2000 in Houston is seeking a Pediatric Physical Therapist to join a mission-driven team delivering home-based therapy to children and families across Greater Heights and surrounding areas.

You will perform evaluations, develop and implement treatment plans, and collaborate with caregivers and healthcare providers to promote each child’s growth and independence. This role offers competitive benefits and a supportive environment.

Qualifications

• 3+ years as a pediatric Physical Therapist
• Experience in a home-health setting
• Specialty Certifications (PCS)
• Bilingual (English/Spanish)

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine patients’ needs for therapy.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of patients’ medical neighborhoods, coordinating care between providers.
• Complete a minimum number of patient visits weekly as required by the agency.
• Meet deadlines for chart updates, progress summaries, call logs, and paperwork.
• Collaborate with agency personnel to ensure understanding of expectations and standards.
• Adhere to agency policies, procedures, and applicable laws.

• -Perform all duties in compliance with agency policies and procedures.
• -Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
