Job summary

KidsCare Home Health is seeking a licensed Physical Therapist to provide pediatric home health therapy. You will evaluate, treat, and educate patients and families in their homes, coordinating with caregivers and healthcare teams.

A strong emphasis is placed on family education, outcome tracking, and maintaining high-quality care. Applicants should have a Master’s in Physical Therapy, valid state license, CPR, and pediatric/home health experience.

Qualifications

• Master’s Degree in Physical Therapy required.
• Current state-issued professional license
• Valid state driver
• CPR certification
• Pediatric clinical experience
• Home health experience

Responsibilities

• Evaluate and administer pediatric physical therapy in patients' homes.
• Train patients and caregivers on home exercise programs and discharge planning.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with patients and families about progress and goals.
• Collaborate with providers and refer to appropriate services as needed.
• Document patient progress and maintain records.
• Maintain supplies and equipment needed for therapy sessions.
• Participate in quality improvement and continuing education.
