Qualifications

• Master’s Degree in Physical Therapy is required.
• Current state PT license is required.
• CPR certification is required.
• Home health experience and pediatric clinical experience are preferred.

Responsibilities

• Evaluate, organize, and conduct pediatric physical therapy in home health settings.
• Provide patient and caregiver training, discharge planning, and progress updates.
• Coordinate care with office staff and other disciplines.
