Job summary

St. Mary’s Healthcare System for Children is seeking a Physical Therapist to evaluate, plan, implement and document home care patient status.

The PT will supervise PTA and cosign their documentation when required, ensuring care aligns with regulatory guidelines and agency standards. Responsibilities include providing skilled PT per plan of care, teaching home exercise programs, ensuring home safety, and coordinating with the team for comprehensive patient care.

Qualifications

• Successful completion of a physical therapy program approved by the APTA or similar body.
• Licensed in NYS as a Physical Therapist.
• Minimum of one year of experience in pediatric physical therapy or pediatric affiliation.

Responsibilities

• Provides skilled physical therapy per the plan of care.
• Supervises PTA and cosigns documentation as needed.
• Evaluates and revises the plan of care based on patient progress.
• Educates patients and families on home exercise programs and safety.
• Coordinates total care with the health care team.
