Job summary

St. Mary’s Healthcare System for Children seeks a dedicated Physical Therapist to evaluate, plan, implement, treat, and document care for home care patients. You will supervise the Physical Therapy Assistant, cosign documentation, and ensure compliance with regulatory guidelines.

You will treat patients using therapeutic activities to remediate musculoskeletal deficits, teach home exercise programs, and participate in care conferences and team coordination for optimal patient outcomes.

Qualifications

• Graduate of an accredited physical therapy program.
• Licensed in NYS as a Physical Therapist.
• Experience in pediatric physical therapy preferred.

Responsibilities

• Provide skilled physical therapy in accordance with the plan of care.
• Teach and supervise patients and caregivers in home rehabilitation.
• Coordinate with RN, Team Supervisor, and other disciplines.
• Maintain documentation and progress notes weekly.
• Ensure home safety and the overall plan of care continuity.
