Job summary

St. Mary’s Healthcare System for Children seeks a dedicated Physical Therapist to perform evaluations, planning, implementation, treatment and documentation. The role collaborates with physicians and a care team to ensure high-quality pediatric rehabilitation.

You’ll participate in care plan meetings, document progress, and contribute to a patient-centered approach. A NYS PT license and BLS are required, with pediatric PT experience preferred.

Qualifications

• Successful completion of a physical therapy program approved by the APTA or AMA programs.
• Licensed in NYS as a Physical Therapist.
• BLS Certification is required.
• Minimum of one year of pediatric physical therapy experience or pediatric affiliation.

Responsibilities

• Provide skilled physical therapy per physician and team plan of care.
• Assess patient functional level using appropriate tests and revise the plan of care.
• Treat patients to relieve pain and restore function using PT modalities and procedures.
• Observe and report patient responses and condition changes to care team.
• Instruct patients, families and health team members in PT phases and use of devices.
• Develop and revise patient care plans, outcomes and discharge plans.
• Initiate diagnostic and rehabilitative procedures; refer to other disciplines as needed.
• Coordinate total plan of care with other health professionals.
