THERAPY 2000 is seeking a licensed Pediatric Physical Therapist to provide home-based therapy to Texas families. You will evaluate children, develop individualized treatment plans, and collaborate with caregivers and physicians to support growth and development.

Qualifications

Current/active license to practice Physical Therapy in Texas.
Innovative spirit with reliability.
Flexibility to shift resources and priorities as needed.
Ability to use common workplace technology (laptop, phone, email, web browser).

Responsibilities

Perform comprehensive Physical Therapy evaluations using standardized tests to determine patients’ needs for therapy.
Develop and implement treatment plans per physician orders.
Develop home exercise programs in partnership with patients’ caregivers.
Act as a vital member of patients’ medical neighborhoods, coordinating care between providers and disciplines.
Meet a minimum number of patient visits weekly as required by the agency.
Complete chart updates, progress summaries, call logs, and paperwork on time.
Meet with agency personnel to maintain understanding of expectations and standards.
Adhere to agency policies, procedures, and applicable laws for home-health pediatric PT.
