Therapy 2000 is seeking a dedicated Pediatric Physical Therapist to provide home-based therapy across Texas. You will conduct comprehensive PT evaluations, develop and implement individualized treatment plans, and collaborate with caregivers to build strong home exercise programs.
This role emphasizes compassionate care, coordination with medical teams, and adherence to all regulations for pediatric home health.
Formación
- Current/active Texas Physical Therapy license
- Innovative spirit and proven reliability
- Flexibility to shift resources and priorities as needed
- Ability to use common workplace technology (laptop, email, browser)
Responsabilidades
- Perform comprehensive Physical Therapy evaluations using standardized tests
- Develop and implement treatment plans aligned with physician orders
- Create home exercise programs with caregivers
- Collaborate with other providers to coordinate care
- Complete a minimum number of patient visits weekly as required
- Meet deadlines for chart updates, progress notes, call logs, and paperwork
- Adhere to agency policies, procedures, and applicable laws for pediatric home health
