Job summary

Therapy 2000 is hiring a Pediatric Physical Therapist to serve Texas families in a home-based setting. You will evaluate children, design individualized plans, and coordinate with caregivers and healthcare providers to drive meaningful outcomes.

The role requires an active Texas PT license, reliability, and the ability to adapt to changing patient needs; travel between patient visits is reimbursed, and the program offers comprehensive benefits.

Qualifications

A current/active license to practice Physical Therapy in Texas.

Ability to use common workplace technology (laptop, phone, email, web browser).

Responsibilities

Perform comprehensive Physical Therapy evaluations using standardized tests to determine patient needs.
Develop and implement treatment plans aligned with physician orders.
Create home exercise programs with patients' caregivers.
Collaborate with other providers and disciplines within the patient’s care network.
Complete a minimum number of patient visits weekly as required.
Maintain timely chart updates, progress summaries, call logs, and related paperwork.
Ensure adherence to agency policies, procedures, laws and ethical guidelines.
