THERAPY 2000 in Texas is seeking a licensed Pediatric Physical Therapist to join our mission-driven team delivering home-based therapy to children across Texas.

You will evaluate, plan, and implement PT services, develop home exercise programs with families, and coordinate care with physicians and other providers, while meeting agency standards and delivering compassionate care in the child’s home environment.

Qualifications

• Must hold an active Texas PT license.
• Innovative, reliable, adaptable to changing priorities.
• Proficient with basic workplace tech (laptop, phone, email, browser).

Responsabilités

• Perform comprehensive PT evaluations using standardized tests.
• Develop and implement treatment plans per physician orders.
• Create home exercise programs with caregivers.
• Collaborate with other providers to coordinate care.
• Complete minimum patient visits weekly as required.
• Maintain patient charts, progress summaries, call logs and paperwork.
• Adhere to agency policies and home-health pediatric standards.
