THERAPY 2000 is a mission‑driven home‑based pediatric therapy provider in Texas. We seek a licensed Pediatric Physical Therapist to evaluate, treat, and guide children in their homes, coordinating with families and healthcare teams.

Qualifications

• Active Texas Physical Therapy license.

Responsabilités

• Conduct comprehensive physical therapy evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with patients and caregivers.
• Coordinate care with other providers and disciplines.
• Meet minimum patient visit targets weekly.
• Complete chart updates, progress summaries, and logs timely.
• Engage with agency staff to align on expectations and quality of care.
• Comply with all agency policies, procedures, and regulatory requirements.
