Job summary

THERAPY 2000 is a mission-driven, home-based pediatric therapy provider serving Texas families. This role focuses on pediatric physical therapy with collaboration across care teams and caregivers to deliver compassionate, evidence-based treatment.

You will perform evaluations, develop plans, and ensure timely visits and thorough documentation while upholding clinic standards and compliance in home-health settings.

Qualifications

• Must hold an active Physical Therapy license in Texas.
• Demonstrated reliability and initiative.
• Ability to adapt to shifting priorities based on business needs.
• Proficient with common workplace technology (laptop, phone, email, browser).

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine therapy needs.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with patients’ caregivers.
• Collaborate with other providers to coordinate care in patients’ medical neighborhoods.
• Complete the minimum weekly patient visits as required by the agency.
• Meet deadlines for chart updates, progress summaries, call logs and paperwork.
• Engage with agency staff to maintain understanding of expectations and standards for high-quality care.
• Adhere to agency policies, procedures and applicable laws in home-health pediatric PT.
