Job summary

Therapy 2000 is hiring a Pediatric Physical Therapist in the Galena Park area to work in a home-based setting across Texas. You will evaluate patients, plan and implement therapy per physician orders, and develop home exercise programs with families and caregivers.

You will collaborate with a broad medical team to coordinate care, ensure timely charting, and maintain high standards of patient outcomes in a home-health environment.

Qualifications

• 3+ years as a pediatric Physical Therapist.
• Experience in a home-health setting.
• Specialty Certifications (PCS).
• Bilingual English/Spanish.

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests.
• Develop and implement treatment plans per physician orders.
• Develop home exercise programs with caregivers.
• Coordinate care with other providers and disciplines.
• Complete a minimum number of patient visits weekly as required.
• Meet deadlines for chart updates, progress notes, and calls.
• Engage with agency staff to meet care standards.
• Follow agency policies, procedures, and applicable laws.

Responsibilities

What you’ll do:

• -Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• -Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• -Develop home exercise programs in partnership with patients’ caregivers.
• -Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• -Complete a minimum number of patient visits on a weekly basis as required by the agency.
• -Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• -Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• -Perform all duties in compliance with agency policies and procedures.
• -Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
