THERAPY 2000 is seeking a Pediatric Physical Therapist for the Alvin/Santa Fe area. This PRN, home-health role involves evaluating and treating children, developing and implementing plans of care, and coordinating with families and other providers to ensure optimal outcomes.

As part of a mission-driven team, you will support a small, consistent caseload, travel between patient visits, and participate in ongoing professional development while delivering compassionate care to Texas families.

Formación

• 3+ years of pediatric physical therapy experience.
• Experience in a home-health setting.
• PCS certification preferred.

Responsabilidades

• Perform comprehensive physical therapy evaluations using standardized tests.
• Develop and implement treatment plans per physician orders.
• Develop home exercise programs with patients' caregivers.
• Collaborate with the care team to coordinate services.
• Meet a minimum number of patient visits weekly as required.
• Maintain timely chart updates, progress summaries and logs.
• Engage with agency staff to meet expectations and delivering high-quality care.
• Comply with all policies, regulations and ethical guidelines.

Qualifications

What you’ll need:

• 3+ years as a pediatric Physical Therapist
• Experience in a home-health setting
• Specialty Certifications (example: PCS)
• Bilingual (English/Spanish)

Responsibilities

What you’ll do:

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
