Job summary

Homeside Rehab is seeking a compassionate Physical Therapist or PTA to join our pediatric home health team in Carle Place, NY. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, helping them reach developmental goals while enjoying schedule flexibility and a supportive team environment.

Responsibilities include evaluating patients, developing plans, implementing PT interventions, documenting care, and coordinating with the clinical team.

Qualifications

• Active NY Physical Therapy License required.
• Strong clinical assessment and communication skills.
• Ability to work independently in a home health setting.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on assessments and goals.
• Provide evidence-based PT interventions to improve motor skills and functional outcomes.
• Document patient care accurately and timely per regulatory requirements.
• Collaborate with clinical team for coordinated treatment.
• Communicate progress and plans with patients, caregivers, and office staff.
