Job summary

New York Community Care is seeking a full-time Physical Therapist for home-based pediatric patients. The role includes evaluation, planning, treatment, and documentation of patient status, with a strong emphasis on quality care and regulatory compliance.

Qualifications

• Successful completion of a physical therapy program at a school approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association, or both.
• Licensed in New York State as a Physical Therapist.

Responsibilities

• The Physical Therapist is responsible for the evaluation, planning, implementation, treatment and documentation of patient status for home care patients.
• Supports organization's mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.

Education

Successful completion of a physical therapy program approved by the APTA or AMA committees, as applicable

Licensed in New York State as a Physical Therapist
