Job summary

Homeside Rehab is seeking a compassionate Physical Therapist or PTA to join our pediatric home health team in Hyde Park, MA. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, offering schedule flexibility and a supportive team environment.

Responsibilities include evaluating patients, developing treatment plans, performing PT interventions, and documenting care in compliance with regulations.

Qualifications

• Massachusetts State Physical Therapy License required (PT/PTA).
• Strong clinical assessment and communication skills.
• Valid MA driver's license and reliable transportation for home visits.
• 1 year of experience in Home Health, Outpatient, or SNF preferred.

Responsibilities

• Evaluate patients and create individualized treatment plans.
• Provide evidence-based PT interventions to improve motor skills and independence.
• Document care accurately and timely per regulations.
• Collaborate with clinical team to ensure coordinated care.
• Communicate progress and care plans with offices, patients, and caregivers.
