Job summary

Homeside Rehab is seeking a compassionate Physical Therapist or PTA to join our pediatric home health team in Hicksville, NY. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, with flexible scheduling and a supportive team environment.

Responsibilities include evaluating patients, creating treatment plans, and documenting care while collaborating with the clinical team. A NY license and transportation for home visits are required.

Qualifications

• Active NY Physical Therapy License (PTA) is required.
• Ability to document results in an electronic system.
• Experience with pediatric home health is preferred.
• Drivers license and reliable transportation for home visits.
• 1+ year in Home Health, Outpatient, or SNF settings.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on assessment and goals.
• Provide evidence-based PT interventions to improve motor skills and independence.
• Maintain accurate documentation of care in regulatory compliant systems.
• Collaborate with clinical team for coordinated treatment.

Job Summary

Homeside Rehab is seeking a compassionate and skilled Physical Therapist or PTA to join our pediatric home health team. In this role, you will deliver individualized, evidence-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Qualifications

• Active NY Physical Therapy License (PTA) — required
• Basic computer proficiency for electronic documentation
• Strong clinical assessment and interpersonal communication skills
• Ability to work independently in a home health environment
• Valid NY driver's license and reliable transportation for home visits
• Pediatric experience is preferred
• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings

Job Details

• Salary: $65 - $80 per hour
• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
