Family of Kidz is seeking a Pediatric Physical Therapist for Early Intervention and Preschool in Baldwin, NY. The role offers a $500 sign-on bonus after 90 days of employment with active cases and a 10-hour minimum weekly caseload.

The position includes designing therapy plans, conducting evaluations, data collection, and collaboration with school teams. Salary ranges per hour vary with experience and licensure. Full-time or FFS options may apply.

Qualifications

• Graduate from an accredited university with a degree in Physical Therapy (DPT).
• Current New York State Physical Therapy License.
• Bilingual is a plus.

Responsibilities

• Design and implement creative individual and/or group therapeutic interventions for all children on your caseload.
• Administer and write up physical therapy evaluations.
• Ongoing communication with lead supervisors & coordinators, specifically related to case management.
• Maintain professionalism with families, school teams, and other professionals.
• Maintain accurate and timely attendance and billing logs.
• Create and maintain a schedule with assigned individual families/school personnel.
• Collect data on each IFSP/IEP goal and reports on progress quarterly.
• Provide input and attend/teleconference initial CPSE/CSE meetings and/or annual reviews.
